Getting To Know Your Insulin Pump

Some health care suppliers like the insulin pump as a result of its slow unharness of insulin mimics how a normally operating pancreas would unharness insulin. Studies vary on whether or not the pump provides higher blood glucose management than multiple daily injections. Another advantage of the insulin pump is that it frees you from having to live insulin into a syringe.

An insulin pump may be a medical device continuously delivering insulin beneath the skin through a catheter. it's always connects somewhere within the waist space. there is a new generation of insulin pumps, referred to as a patch pump. Currently patch pumps are solely on the market from OmniPod. Patch pumps adhere on to the skin with no catheter tubing showing. It then infuses insulin directly beneath the skin.

Either pump delivers insulin at an hourly rate. as an example, the speed may well be one.1 units an hour. However, the pump delivers completely different|completely different} rates at different times of day counting on the patient's insulin infusion (or basal) rates that are programmed into the pump.

The amount of insulin delivered depends on 2 things. 1st by the quantity of carbohydrate a patient eats using an insulin to carbohydrate ratio, and then by the correction issue, or the ratio of the quantity of milligrams per deciliter (mg/dl) a patient's blood sugar are lowered by one insulin unit.
If a patient eats sixty grams carbohydrate at meals and has an insulin-carbohydrate ratio of 1 insulin unit to fifteen grams carbohydrate, the patient's insulin injection at that meal would four units.
However, if a patient features a correction issue of 1 unit to fifty points of blood sugar, the pump ought to provide a further injection of two.5 units to lower his blood sugar from 245 mg/dl to a required level of one hundred twenty mg/dl.

To use an insulin pump a patient should be able to manage it. This involves information at many levels. First, patients should perceive the way to insert the catheter when using the pump, or the way to attach the newer patch pump to their abdomen. they need to even be able to push correct|the correct} buttons on the pump to deliver proper insulin doses and alter the basal rates.

Then the patient has to be skilled in carbohydrate counting so that they are able to deliver correct insulin doses at mealtimes. and that they ought to be willing to visualize their blood glucose levels a minimum of four to 6 times each day. This assures that they detect a pump failure and forestall hyperglycemia and diabetic ketoacidosis (DKA, in kind one patients).

Patient attention is very important as a result of no long-acting insulin is employed in kind one patients who use pumps and that they have to be compelled to correct high- or low-blood sugars before they're clinically observable and symptomatic.

Insulin pump therapy is sort of never required to keep up life as a result of insulin is simply injected beneath the skin. Most insurers can cowl insulin pump therapy in things where insulin pump therapy can considerably improve the amount of diabetes care and management over and higher than multidose insulin (MDI) therapy. This includes cases where:

The glucose management in multidose insulin therapy isn't optimal with glycated hemoglobin (Hba1c>) than the ADA (American Diabetes Association) counseled goal of seven. An endocrinologist, who are able to facilitate the patient learn the way to use and also the pump and alter basal and correction doses, prescribes the pump.

The patient has kind one diabetes. However, in several things patients with kind two diabetes can take pleasure in the pump furthermore. Presence of hypoglycemia despite changes in insulin doses and utilizing carbohydrate counting to assist decide pre-meal insulin doses in patients who are using MDI therapy.

Presence of hyperglycemia-especially as revealed by high morning readings (Dawn phenomenon) where increasing basal rates of insulin within the early morning hours would facilitate to higher management blood sugar levels.

Insurers need medical charts from the prescribing doctor furthermore as blood sugar logs from the patient to prove that there's real medical necessity.

Indian doctors needs exposure to Insulin pumps

Diabetic population in India is nearly 400 million, and considering an estimate of 14 percent patients on Insulin can be translated to 56 million Insulin treated patients in India. Out of these at least 10 percent that is 5.6 million patients most likely are having very high glucose levels and grossly uncontrolled Diabetes. Further at least 5 percent that is 2.3 million patients will not be in good control and are in search of options which can allow them to have a flexible life style and still good control.

These are patients who need a better Insulin treatment strategy. Such strategy is available in India since last 6 years. It is known as Insulin pump, which is a small pager sized device, and this device can replace Insulin like pancreas.

In last 6 years, number of Insulin pump users in India is estimated to be 1500 which is too small. Since pump is a costly therapy so a limited number is expected to use it but as compare to Insulin treated population, pump user population is too small indicating

According to endocrinologist Dr. Sunil M Jain, cost factor does play a role but additionally there are many other hurdles for pump therapy in India:

1. Lack of correct information among health care providers

2. Self testing of blood glucose is low

3. Myths about pump

4. Non availability of paramedics taking care of pump problemsConcept of health economics is missing

5. Many type 1 Diabetics are not aware about its availability

Many patients feel that pump requires a surgery. While pump is a device which can be carried in pocket and a tube is attached to skin and through this Insulin reaches in our body. Many doctors are not aware about pump menu, pump software and thus they try to avoid prescribing pumps. Even if a doctor is aware of pump and its use, often there is no time to explain and demonstrate it to patients. Thus in India there is a need to train doctors, as well as paramedical staff about pump use. Once doctors are trained and able to mange the device then only pump usage will raise. In developed countries, nurses often put patients on pump and do most of the management. But in India, patients are comfortable if initial management is done by doctor follow up management by assistant. Thus for India, there is need to train doctors as well as paramedical staff and diabetes educators. This may change the paradigm of management of Diabetes and there may be a major change in quality of life of all type 1 Diabetics and type 2 Diabetics on MSI regimen of Insulin.

What are the Pros and Cons of Using Insulin Pumps

As a means of coping with diabetes mellitus, there are different insulin therapies that one can go for, and using insulin pumps are just one of these. Insulin injections are the others, but in this context, insulin pumps are discussed, together with their accompanying advantages and disadvantages. Usually, insulin pumps come with a couple of things; a pump which is battery-powered, the insulin contained in a kind of reservoir like a cartridge, and the set that is inserted under the skin. They provide insulin on demand, which is often associated with more freedom when it comes to the dietary choices.

For starters, insulin pumps eliminate the need for insulin shots using an insulin pen or a needle. Those who desired more freedom from taking frequent shots and the trypanophobic would welcome the use of insulin pumps. That's because an insulin pump will only need to be changed every couple of days. You also get relief from any bouts of hyperglycemia as the insulin pumps provide insulin 24 hours each day. This means that your glucose levels will always remain normal, so long as the insulin pump is working.

Insulin pumps generally ease the management of Type 1 and Type 2 diabetes which needs insulin administration. On top of that, they provide more accurate insulin administration when compared against injections which are more likely to over or under-estimate the dosages needed. Neuropathy, a condition affecting those with treatment resistant diabetes experience much lesser pain when they start to use insulin pumps.

However, it is not all good news with insulin pumps as there are some associated disadvantages. It is a more expensive approach of insulin therapy when compare against insulin shots, since there are the costs of the pumps and infusion sets that need to be considered. Since the insulin shots are worn all the time, they are rather limiting when it comes to indulging in some activities, like swimming, or playing an intensive sport. Overtime, it can get rather uneasy to have them around.

Insulin pumps are electronic devices after all and could break down or malfunction. When this happen, you have no choice but use insulin injections till your pump is replaced. Also, should the pump run out of power, and insulin is not administered into your body, the resulting insulin deficiency may lead to diabetic ketoacidosis (DKA), as the body tries to compensate by using ketones for energy. As a precaution, people who used insulin pumps are required to monitor their glucose level more frequently so as to make sure the pump are functioning. They do have their shortcomings, but the advantages that the insulin pumps greatly outweigh their disadvantages according to most users.

The other insulin therapy methods also have their strengths and weaknesses. Deciding on the type of insulin therapy largely depends on your personal preference. No one method is right or wrong and there are many people who swear by either methods too. At the end of the day, what's most important is that the therapy you are using gives you the desired results.
